Purchase a PLA450 HomePlug AV Powerline Wireless Access Point at
any Participating Retailer and Receive $20 USD via Mail-In Rebate
Offer is good for purchases made between 1/01/08 and 1/31/08. Rebate
submission must be postmarked by 3/31/08. Limit five (5) rebates per
household. Read below for Terms and Conditions.
1. Purchase the PLA450 between 1/01/08-1/31/08.
2. Read and complete this rebate request form completely, as incomplete forms will
not be accepted.
3. Mail the completed form with the original UPC code from the package. The UPC
code is a 12-digit bar code on the bottom of the package.
4. Enclose a copy or original store identified dated sales receipt. Handwritten or
altered documents will not be accepted.
5. Keep copies of all rebate materials submitted; originals submitted become ZyXEL
property and will not be returned.
